Use this if you have a guiding role with the Department of Health, a partner organization, a Local Government Unit, or a private provider network. This app is designed to make reviews (on-site or remote) efficient and effective.
With this app, you can:
• Retrieve electronic versions of standard monitoring tools, program review questionnaires, and other TB survey tools
• Complete forms digitally off-line (no internet connection needed) and submit to central server for filing and storage
• View the latest accomplishments as reported in the Integrated TB Information System (ITIS)
• Measure progress against previous recommendations
• Send assessment reports immediately with your findings and recommendations
If you supervise assessors (requires ITIS log-in with supervisor role), you may also use the web app at www.leadtb.doh.gov.ph to design custom forms, download results, and follow plans and progress through the online dashboard.
